Your medical paperwork must be completed by July 1st no matter what sport you are participating in. If your medical paperwork is incomplete, you will not be allowed to start practice.

Frequently Asked Questions

Do I need to provide documentation for sickle cell?
Yes, we require that you have documentation of a sickle cell trait results. Here is a link to find the contact information to each state health department as well as the start dates for sickle cell testing in each state - LINK.

What is my athletic ID #?
You are given a number ID from the school and that is your Athletic ID#. Please do not put a random number or ID into the ATS system.

I can’t log in to the ATS?
If you are an incoming student-athlete or transfer you need to login in as ID# new and password new. For the returners, you need to enter your student ID# and bearcats. If you are still having problems contact Ken Smith.

I try to login but it won’t accept my password?
All student-athletes have their password changed each year to bearcats. If you are still having problems contact Ken Smith.

Insurance card photo size?
The insurance card photo size needs to be less than 1 MB. You can decrease the pixels and/or size of the photo. You can also crop the size of the photo too.

I am hitting the save button and the webpage goes blank?
The most likely reason for this is your insurance card size is over 1 MB.

Do I need a physical? When can I get it done?
Yes, you only need to have your physical 1 time at Willamette. You can get this done June 1st or after of this year for incoming student-athletes and transfers. Remember to take a completed health history form with our physical form.

What do I need to take to the doctor?
You need to take a completed health history form with our physical form. You only need to take the ban substance form if you are taking any substances that are banned.

Can anybody sign my return to play for a surgery or condition?
No, the surgeon or specialist needs to sign your medical clearance to participate in athletics at Willamette University.

Where can I find the ImPACT directions, Physical Evaluations, Banned Substances form, and Return to athletic competition form?
When you are in the ATS system, go to the e-files tab on the far right. Click on the tab and you will be able to view and print all the forms.

Who has to take the Impact test?
The Impact test is required for all first-year and transfer students  Football student-athletes have to take the Impact test every year.
